By signing this media release form, you grant 901parties Dining and Catering permission to use photos, videos, audio recordings and all media captured of you, your family and those in your party for promotional and advertising purposes.

This form provides 901parties with the flexibility to use media content across various platforms while ensuring consent from participants.

   I, the undersigned, grant 901parties Dining and Catering and its representatives the right to record my image, voice, and likeness through photography, videography, and audio recording (collectively, the “Media”). This consent includes:

### 2. **Purpose of Use**

   I understand that the Media may be used by 901parties Dining and Catering for promotional and advertising purposes, including but not limited to:

   - Social media platforms (e.g., Instagram, X, YouTube, TikTok Facebook, Twitter, LinkedIn, etc.)

   - Television commercials and digital advertisements

   - Radio broadcasts and podcasts

   - Websites and blogs

   - Printed promotional materials

   - Any other media outlets, both digital and traditional

### 3. **Duration of Use**

   I understand and agree that this consent is granted in perpetuity.

### 4. **Waiver of Compensation**

   I understand and agree that I will not receive any payment, royalties, or compensation for the use of the Media, now or in the future. My participation is voluntary, and I understand that 901parties Dining and Catering reserves the right to use or refrain from using the Media at its discretion.

### 5. **Waiver of Rights**

   I waive any rights to inspect or approve the Media or any finished product in which the Media may be used. I also waive any right to royalties or other compensation arising from or related to the use of the Media.

### 6. **Release from Liability**

   I hereby release, discharge, and agree to hold harmless 901parties Dining and Catering, its employees, agents, representatives, and all persons acting under its permission or authority, from any liability arising out of or in connection with the use of the Media, including but not limited to any claims for defamation or invasion of privacy.

WAIVER AND RELEASE FROM LIABILITY

### Waiver and Release of Liability

**Event Activity Waiver and Release for Face Painting and Latex Balloon Animals**

(the “Participant” or “Participant’s Parent/Guardian” if Participant is under 18 years old) in favor of **901PARTIES** and its employees, contractors, volunteers, and affiliates (collectively, the “Releasees”).

By signing this Waiver, the Participant (or Participant's Parent/Guardian) acknowledges, understands, and agrees to the following:

### 1. **Acknowledgment of Risks**
- **Face Painting**: I understand that face painting involves the use of hypoallergenic paints and cosmetic-grade materials; however, some individuals may still experience reactions, including but not limited to skin irritation, allergic reactions, and other sensitivities.

- **Latex Balloon Animals**: I understand that latex balloons may cause allergic reactions in some individuals and may also pose a choking or inhalation hazard, especially for young children.

### 2. **Assumption of Risk**
I, the undersigned, understand that participating in face painting and balloon animal activities involves certain inherent risks, including but not limited to the risks of skin reactions, allergic reactions, physical injury, and the risk of balloon deflation or breakage that could pose hazards.

I assume full responsibility for any injuries or damages that may occur as a result of my (or my child’s) participation in these activities and understand that such risks are fully understood and accepted.

### 3. **Release of Liability**
I, the undersigned, hereby release and hold harmless 901PARTIES, its employees, contractors, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or harm that may result from my (or my child’s) participation in face painting and/or balloon animal activities, whether caused by the negligence of the Releasees or otherwise.

### 4. **Medical Treatment**
In the event of any injury or allergic reaction, I give permission for 901PARTIES staff to provide or seek first aid or other medical treatment, as they deem appropriate, and understand that I will be responsible for any associated costs.

### 5. **Photo and Media Release**
I grant permission to 901PARTIES to take and use photographs or videos of the Participant during the event, including images of face painting and balloon animals, for promotional purposes, social media, or other uses as outlined in 901PARTIES’s media policy. I understand that no compensation will be provided for the use of these images or videos.

### 6. **Binding Agreement**
This Waiver shall be binding upon the Participant, his/her heirs, legal representatives, and assigns. I certify that I am signing this document voluntarily, fully understanding its content and significance.
